Parents need to complete the Covid Waiver for each player registered with Fairport Soccer Club.  The Player Family Covid Link will allow you to loop through each of your participating children.

If you are a coach (parent or contracted),  there is a manual form at the bottom which must be printed out and signed and emailed to the FSC REGISTRAR.  This will be sent to RDYSL/NYSW as your/your team's proof of signing the waiver. 

To print out the document, hover over the bottom of the last page of the document and the print option will appear.

Please fill out this form by March 19, 2021.  This form MUST be filled out for each player in order to receive team player passes from RDYSL.
